Graduating members may extend their coverage for 12 consecutive months by applying for extended coverage within 35 days of termination of your GSU insurance (end of term). Some benefits are reduced if you move outside Ontario and it is non-refundable.

Final Year PhD students: You will be charged monthly tuition fees and non-refundable 1-term ancillary fees, which include both health and dental insurance. You will be insured from September to December. If you don’t complete by January, you will then be charged non-refundable 1-term ancillary fees which include this health insurance, and be insured until August 31. At the end of the final term you were charged ancillary fees for, you may purchase the “12-month Extended Insurance.”
Application Periods
Sept. 1st – Oct. 7th, 2011. No exceptions will be made.
How To Apply
In order to purchase the 12 month extension, you must:
1. Fill out an application form, available below and in the GSU lounge during the advertised opt-in period only. You must apply within 35 days of termination of your GSU insurance (end of term).
2. Submit the completed application form and a cheque or money order (made out to the Graduate Students’ Union) by the posted deadline.
