APPLICATION PERIODS:
THE FALL 2009 APPLICATION PERIOD ENDED OCT. 9TH. NO EXCEPTIONS WILL BE MADE.
Students starting in January 2010: January 5th - February 4th, 2010.

How To Apply:
In order to add family members to your plan, you must:
1. Fill out a GSU application form, available below and in the GSU lounge during the advertised opt-on periods only. You must apply in the first term in which you are registered, or when you lose your other coverage (proof required).
2. Submit the completed application form and a cheque or money order (made out to the Graduate Students' Union) by the posted deadline.
