Extended Health & Dental Insurance Information:

 

Graduating members may extend their coverage for 12 consecutive months by applying for extended coverage during the Change of Coverage period only after termination of your UTGSU insurance (end of term). Some benefits are reduced if you move outside Ontario and it is non-refundable.

 

Final Year PhD students: You will be charged monthly tuition fees and non-refundable 1-term ancillary fees, which include both health and dental insurance. You will be insured from September 1st to December 31st. If you don’t complete by January, you will then be charged non-refundable 1-term ancillary fees, which includes health and dental insurance, and you will be insured until August 31st. At the end of the final term in which you were charged ancillary fees, you may purchase the “12-month Extended Insurance.”

 

Application Periods:

 

Students whose coverage ends August 31, 2015: September 1st – September 30th, 2015

Students whose coverage ends December 31, 2015: January 2nd – January 22nd, 2016

Complete the online Application Form during the advertised opt-in periods only. Please visit the studentcare.net/works website, www.ihaveaplan.ca for the rates and applications forms. You must apply during the Change of Coverage period only after termination of your UTGSU insurance (end of term).
